Passport to the well is a document containing information on the technical condition and parameters of a well, including its geological and hydrogeological characteristics. According to the Resolution of the Cabinet of Ministers of Ukraine №963 dated 08.10.2012, the State Service of Geology and Mineral Ressources of Ukraine carries out state registration of wells.

Water well passport: what are the main features of the document?
The water well passport contains information about the well designer, drilling organisation, description of the well parameters, construction data, technical condition, observation results and hydrogeological indicators, etc.
To develop an artesian well passport, you need to have all the information about the well.
Location and geographical coordinates of the well with an accuracy of 1 second;
Ownership of an artesian well by a legal entity or an individual;
The purpose of an artesian well;
Chemical composition of water (mineralisation level, presence of harmful impurities, etc.);
The productivity of a well (the amount of water it can produce in a certain time);
Technical condition of the well equipment (condition of pumping equipment, availability of filter materials, condition of pipelines, etc;)
Depth of the well;
Diameter of the well;
Water level in the well.
Artesian well passport: development
The artesian well data sheet is developed by a specialised drilling organisation based on the data obtained during the drilling of the well. If the drilling organisation is unable to determine the water parameters independently, then the water parameters shall be determined by an accredited laboratory. Requirements for determining sanitary and chemical parameters and epidemiological safety of drinking water are determined in accordance with the State Sanitary Norms and Regulations "Hygienic Requirements for Drinking Water Intended for Human Consumption", approved by Order of the Ministry of Health of Ukraine №400 dated 12.05.2010, registered with the Ministry of Justice of Ukraine on 01.07.2010 under №452/17747.
The technical passport for the well shall contain a 1:25000 scale overview map with the drilled well, a well description that includes all general data on the well. Geological cross-section and design of the artesian well, as well as the results of research based on the results of trial (test) pumping of water from the artesian well.
In addition, a number of analyses are required to determine sanitary and chemical indicators of water safety and quality, as well as radiation indicators of drinking water safety. The chief engineer describes the hydrogeological conclusion in the passport, if there are any superstructure facilities, sanitary regime zones and information on the repair of the artesian well.
